Position Overview
We are seeking an experienced MDS Coordinator to oversee the MDS process, ensure accurate clinical assessments, and support reimbursement optimization within the facility. Strong candidates will maintain compliance with state and federal regulations while collaborating closely with interdisciplinary teams to support quality resident care and operational performance. 

This position requires a detail-oriented professional who can perform effectively within a fast-paced healthcare environment while maintaining accuracy, organization, and strong communication. 

Location: Rochester, NY 
Work Setting: Onsite 
Employment Type: Full-Time 
Salary Range: $90,000 – $120,000 USD/Annually

Key Responsibilities 

  • Coordinate and complete MDS assessments in accordance with state and federal regulations 
  • Ensure timely and accurate MDS submissions 
  • Collaborate with nursing, therapy, and interdisciplinary teams on care planning 
  • Support Medicare and Medicaid reimbursement optimization 
  • Monitor documentation accuracy and compliance standards 
  • Participate in audits, surveys, and quality improvement initiatives 
  • Maintain current knowledge of CMS guidelines and reimbursement practices 
  • Assist with care plan development and resident assessment coordination 
  • Ensure accurate clinical documentation to support reimbursement and compliance objectives 
  • Communicate effectively with facility leadership and clinical staff regarding assessment updates and requirements 

Qualifications 

  • Active RN license preferred; LPN candidates with strong MDS experience may be considered 
  • Previous MDS experience within skilled nursing or long-term care settings 
  • Strong understanding of Medicare, Medicaid, CMS regulations, and reimbursement processes 
  • Excellent organizational, communication, and documentation skills 
  • Ability to multitask and manage deadlines effectively 
  • Strong attention to detail and regulatory compliance standards 
  • Ability to collaborate effectively within interdisciplinary healthcare teams 

Compensation
The published salary range is used as a guide to provide prospective candidates with a level of compensation that is competitive with today’s market. The salary range for this position is a good faith estimate that allows for flexibility to align with various levels of experience, education, and performance.
